Usual adult dosage is 1 tablet taken orally once daily or as prescribed by a licensed medical practitioner. Dispensed by Prescription.

Folarex™ is supplied as Multivitamin in a tablet, 30 Tablets, NDC 85477-511-30.

Yes — Folarex™ is Rx Only and must be prescribed by a licensed healthcare provider and dispensed by a licensed pharmacy.

Store at 20°–25°C (68°–77°F); excursions permitted to 15°–30°C (59°–86°F) [See USP Controlled Room Temperature]. Avoid excessive heat, light, and moisture. Folarex™ ships from Oncora Pharma's Dallas, TX distribution center to licensed pharmacies and wholesalers.

Do not use if you are allergic to any of the ingredients, and keep out of reach of children. Stop use and contact a healthcare provider if irritation develops or symptoms persist.
